During your first office visit we will spend 30-90 minutes, depending on the nature of your chief complaint, obtaining a thorough health history, reviewing your concerns and goals, and creating a tailored health plan. We believe that by understanding a person's whole story we can better determine the root cause of a particular issue, or issues, and develop a personalized health plan that will work for each individual's unique challenges and lifestyle.
It is our goal to make sure you feel like you have everything you need to feel empowered on your new health journey, which is why we always include detailed instructions and additional educational resources to aid in your success. With convenience in mind, we are also able to provide follow-up phone and virtual (video) consults for those who are traveling or live out-of-town.

Please complete the client forms (below) and bring a copy of any recent lab work or medical records you would like to have reviewed. Follow up visits will be 30 to 60 minutes, depending on your needs.
Therapeutic modalities we use include therapeutic nutrition, lifestyle coaching, botanical medicine, hydrotherapy, supplementation, and homeopathy.

Completed new client paperwork.
Pertinent medical records including recent labs or imaging reports. If you do not have these, you can have them sent by downloading and completing the records release form.
The following paperwork is designed to provide a clear picture of your past and current health, to help us spend more time talking about what is pertinent including designing an individualized and comprehensive health plan for you. It can take awhile to complete the paperwork, be sure to give yourself enough time.
If you do not have access to a printer, please call the clinic and we will be happy to send you a copy in the mail. Be sure to complete steps 1 and 2 (step 3 optional) at least 24 hours prior to your visit.

3. If you have any recent lab work, imaging, or medical records please fax, email, or mail those to us prior to your appointment.
We accept cash, check (with photo ID), and all major credit cards (Visa, Mastercard, American Express, and Discover). Payment is due in full at the time of visit. Unfortunately, insurance does not cover our services.
As last minute cancellations are difficult to fill, please give 24 hours advance notice of cancellation. If you cancel less than 24 hours before your appointment, you will be charged a fee of $50.
