IBS is not a root cause: Here’s What It Actually Means

IBS is not a root cause, it’s a starting point.

If you have been told you have IBS (Irritable Bowel Syndrome), you’re not alone. For many people this diagnosis, doesn’t feel like an answer at all - more like a label.

What Is IBS: Really?

IBS is a diagnosis based on symptoms, not a single identified cause. It is typically defined by bloating, abdominal pain, and diarrhea, constipation, or both.

Unfortunately, IBS does not explain why these symptoms are happening, it simply describes the pattern.

Why IBS Is Not a Root Cause

Being told you have IBS often means no structural disease was found, standard labs and imaging were normal, and symptoms persist without a clear explanation - this is often why IBS is called a ‘catch-all diagnosis.’

But symptoms like bloating, gas, and irregular bowel movements, don’t happen randomly, there is often an underlying driver.

Common Root Causes Behind IBS Symptoms

At Sun Valley Natural Medicine, we look deeper to discover the root cause and develop an individualized approach.

Some of the most common contributors include:

  • Gut imbalance (dysbiosis)

  • Small Intestinal Bacterial Overgrowth (SIBO)

  • Food sensitivities

  • Inflammation

  • Stress and nervous system dysregulation

  • Impaired digestion or motility

IBS vs SIBO: What’s the Difference?

This is one of the most important distinctions we can make, since many people are diagnosed with IBS, but may actually have SIBO. This means treatment approach changes as well.

IBS

  • Symptom-based diagnosis

  • Broad and non-specific

SIBO (Small Intestinal Bacterial Overgrowth)

  • Identifiable condition

  • Bacteria in the small intestine where they shouldn’t be

  • Often causes bloating, gas, and bowel changes

Testing Options (When Appropriate)

Testing isn’t always required, but it can provide clarity when needed. There are a few different types of tests done to fully evaluate an individual with no known root cause such as:

  • Comprehensive Stool Testing (GI Effects, Tiny Health) - These do not diagnose a particular disease but help with understanding function much better to further assess: gut bacteria balance, inflammation, and digestion and absorption.

  • Trio-Smart SIBO Breath Test - A 3-gas, at-home breath test that measures Hydrogen, Methane, and Hydrogen Sulfide to diagnose Small Intestinal Bacterial Overgrowth (SIBO) and related methane/sulfide overgrowths.

  • IBS-Smart Testing - A blood test that can help identify post-infectious IBS and one of the few tests that can support an IBS diagnosis

How We Approach Healing

At Sun Valley Natural Medicine we don’t stop at the label, instead we focus on restoring how the gut functions. One framework we often use is the 5R approach:

Remove - Identify and reduce triggers:

  • Problematic foods

  • Infections or overgrowth

  • Irritants

Replace - Support digestion:

  • Stomach acid

  • Digestive enzymes

  • Bile support

Reinoculate - Restore beneficial bacteria:

  • Targeted probiotics, synbiotics, or sporebiotics

  • Prebiotic support (when appropriate and tolerated)

Repair - Heal the gut lining:

  • Nutrients like glutamine, marshmallow, butyrate, and more

  • Anti-inflammatory support

Rebalance - Address lifestyle factors:

  • Stress

  • Sleep

  • Nervous system regulation

A Different Way to Think About IBS

Instead of asking, ‘is it IBS?’, consider asking ‘what is driving these symptoms I am having?’ The answer to this question changes everything.

If you’ve been given an IBS diagnosis but still don’t feel better, you’re not alone and you are NOT out of options. With the right approach, it’s possible to understand what’s actually happening in your gut and begin to heal.
